Following approval at the Integration Joint Board (IJB) on 30th August 2022, Aberdeen City Health and Social Care Partnership (ACHSCP) is issuing a tender for the Link Practitioners Service. This service will provide a social prescribing service to those presenting at primary care services with socio-economic challenges that are impacting on their health.
