ACHSCP and Bon Accord Care Achieve Digital Telecare Award

Digital Telecare for Scottish Local Government were delighted to confirm that Aberdeen City Council Health and Social Care Partnership and Bon Accord Care have been awarded the Bronze Digital Telecare Implementation Award in recognition of the progress they have made on their analogue to digital telecare transition project.

Adult Support and Protection biennial report 2020/22

A campaign has been launched by Aberdeen City Alcohol and Drugs Partnership (ADP) to help prevent avoidable deaths and hospitalisations from drug overdoses across the city.

Scottish mental health charity SAMH will continue to deliver the Aberdeen City Link Practitioner Service
until 2027.

NHS Grampian is to provide care management teams with direct access to healthcare records, to improve care planning for people across the region. At present, social care teams in Aberdeen City, Aberdeenshire, and Moray must request information from NHS staff. During the trial, care management teams will be able to access the data – stored on a system called TrakCare - directly.

The Independent Food Aid Network (IFAN) has collaborated with local authority teams, advice providers, food aid organisations and other local stakeholders to co-produce cash first referral leaflets as straightforward resources both for people facing money worries and support workers. 

The Aberdeen City Integration Scheme is required under the Public Bodies (Joint Working) (Scotland) Act (2014).   It details the various governance arrangements that exist between the Integration Joint Board and the two parent organisations – Aberdeen City Council and NHS Grampian.  

A new Carers Strategy for Aberdeen is now out for public consultation.

September is Foetal Alcohol Spectrum Disorder (FASD) Awareness Month and Aberdeen City Health & Social Care Partnership wants to remind people that alcohol exposure during pregnancy is the cause of FASD which is a lifelong, ‘hidden disability’ that affects 1 in 20 children born in Scotland.

Following approval at the Integration Joint Board (IJB) on 30^th August 2022, Aberdeen City Health and Social Care Partnership (ACHSCP) is issuing a tender for the Link Practitioners Service. This service will provide a social prescribing service to those presenting at primary care services with socio-economic challenges that are impacting on their health.

Menopause support in Aberdeen

Calsayseat Medical Group is delighted to advise that they are working with River Dee Medical Group to explore ways to develop their practices for the future.

During the pandemic, the Scottish Government issued guidance, recommending patient moves between GP practices be paused, except where individuals/households had moved outwith practice boundaries or were new arrivals to the area. From 1 June 2022 this guidance will be withdrawn.

Dementia Awareness Week – Monday 30 May - Sunday 5 June

People who develop early onset dementia in their 40s, 50s or early 60s, or sometimes younger, often have different needs from older people. For example, they may still be working when they are diagnosed, they may have a young family, and their financial situation may be different.

Bon Accord Care Occupational Therapy (OT) department has played a key role in Bill and Anne’s dementia journey over the past three years, enabling Bill to stay at home for as long as possible.

Families and friends trying to provide support and care for a person with dementia can find it devastating. That person may be a parent, spouse or friend who has been known and respected and it is difficult to watch the deterioration of someone who has been the mainstay of a family.

Date : Monday 30 May - Sunday 5 June

The aim of the week is to raise awareness of dementia and help improve the lives of people with dementia, their families and carers. This year's theme is 'Let's Prevent, Care, Cure dementia together'.

Collaborative Commissioning Workshops

The Aberdeen City Links Practitioners Service contract is due to end on 31 March 2023. Aberdeen City Health and Social Care Partnership is undertaking a collaborative commissioning process to develop the tender specification document for the new Links Practitioner Service contract.

The consultation period for Aberdeen City Health and Social Care Partnership’s (ACHSCP) Strategic Plan for 2022 to 2025 has been extended to Monday 04 April 2022 to allow further public participation.

A plan to transfer four GP practices in Aberdeen from being run by Aberdeen City Health and Social Care Partnership (ACHSCP) to being independently managed by GPs reached fruition on Tuesday March 01.

The Nurse Practitioner service at the Healthy Hoose, which has been one of the services on offer to the communities of Middlefield, Cummings Park, Northfield and Heathryfold in Aberdeen for more than 20 years, is to temporarily close on Tuesday 01 March 2022.

An Aberdeen partnership has recently launched a new project to draw attention to the importance of strength and balance training for preventing falls. 

To mark National Adult Support and Protection Day 2022 (Sunday 20 February) people in Aberdeen are being asked to keep an eye out for things that don’t look right, and to report concerns as an adult protection referral.

The Keep Growing Aberdeen Project (KGA) evaluation report sets out the evidence for the impact of the project in the delivery of local priorities aimed at:

• Improving physical health and mental wellbeing of people,
• Maximising disused outdoor space to increase food growing opportunities,
• Supporting local volunteering opportunities beyond the pandemic and improving community food growing spaces by 2023.
