The Aberdeen City Health & Social Care Partnership (ACHSCP) published a strategic plan for the years 2022-2025, after it was approved by the IJB. The strategic plan outlines our ambitions for the adult health and social care services which were delegated by Aberdeen City Council and NHS Grampian to the ACHSCP.
It reflects the many conversations we have had, with the people of Aberdeen and our professional colleagues across all sectors and services, about what health and social care integration means to them and the services that they value highly.
Our focus will always be on improving the health and wellbeing of our citizens and seeking to reduce the health inequalities that exist in our city.
